Prevalence of periodontal disease among the population is very high (about 90 %), so it is important to during the clinical examination to pay attention to it and inform the patient. Gum bleeding is one of the main symptoms of gingival and periodontal inflammation. For the objective assessment of the pathology, periodontal pockets should be measured, periodontal indices calculated, radiological examination and other tests performed (Picture 1.). Dental plaque is the initiating factor of periodontal disease. The inflammation can result in loss of the tooth supporting tissues, resulting in tooth mobility. If untreated, the disease can cause the loss of teeth (Picture 2.).

In simple cases, a good professional and personal hygiene will be enough, in complex cases – deep pockets cleaning (curettage) or flap surgery with or without guided tissue regeneration (involves gingival flap elevation, inflammatory tissue removal and suturing) is needed. In all cases, in order to ensure the long-term results of treatment very good personal hygiene is a must.

Gum recession cause denudation of roots of the teeth, which increases tooth sensitivity, make the margin of the restorations (fillings, crowns) visible, causes esthetic defects. In these and other cases, the various periodontal plastic surgery procedures (Picture 3) are carried out. Extremely big recession defects cannot be covered in a predictable way.

Dental implants require more careful personal hygiene than natural teeth. Incorrectly cleaned prostheses on implants can cause peri-mucositis (inflammation of the gums around the implant) at the beginning, which can then develop to the peri-implantitis (implant bone loss and inflammation around the implant). Prognosis of peri-implantitis is worse than that of periodontitis (Picture 4), as self-defense mechanisms around dental implants are weak.

Therefore, patients should be informed on adequate prosthetic implant hygiene and be under regular control after prosthodontic phase of treatment.
