Dental implants for edentulous patients

World Health Organization (WHO) considers the loss of some or all teeth as a disability due to the loss of the very important chewing function. Furthermore, tooth loss has a profound effect not only on chewing, but also on digestion, social life, appearance and speech. If these cases are solved using conventional complete removable prostheses (dentures), it increases the bone resorption (bone loss) and there are fewer possibilities to perform implant treatment later on.

We fully inform our patients about tooth preservation, the consequences of tooth loss and prosthetics. If the condition of the remaining teeth is good and they are suitable for prosthetics, we always recommend keeping them. 

The most common methods used during implant treatment aim to restore the chewing function and other functions.

The main options of implant treatment to restore chewing function in patients with partial or complete tooth loss are:

All-on-X  and non-removable prostheses and TFW protocol

In an edentulous jaw, a non-removable (fixed) prosthesis might be supported by 4-6, sometimes 8 or more implants. This treatment method ensures highest chewing efficiency and aesthetics, since the restoration is supported by the implants and resembles the natural teeth as much as possible. The number of implants depends on the patient’s anatomy, bite, bone qualities and other factors. The lowest possible number is four implants (All-on-4 is one of the currently most popular methods). These days the technique is called “All-on-X” without specifying the number of implants.  Temporary prostheses may be provided immediately after implant placement. Before the procedure, thorough examination is necessary, as well as treatment planning, 3D computed tomography, surgical guide preparation and other setup steps.

Scientific studies and experiments done over several years lead to the creation of TFW (Total Fit Workflows) protocol, which ensures more accurate implant placement (less bone augmentations), allows for the placement of non-removable temporary prosthesis immediately after implant placement, easier healing and a more accurate final restoration, which may be manufactured from higher quality and more biocompatible materials (eliminates use of metal alloys).  This procedure requires additional solutions, such as 3D image alignment algorithms and specific prosthetic components. The accuracy of final restorations is evaluated with proprietary devices developed in-house. Therefore, TFW protocol ensures a more predictable treatment and reduces the number of complications, which are quite common in this type of procedure.

Removable prosthesis on 2-4 implants

A fixed (non-removable) restoration on four implant may not be possible due to unfavorable implant placement, reduced diameter or length. In these cases, 4 or only 2 implants may be used to improve removable denture retention.  This solution provides good aesthetics and exceptional stability (removable dentures are a lot more stable, especially with 4 implants). Furthermore, the dentures are easy to maintain, because they are cleaned outside of the mouth; however, implant maintenance is a bit more complicated. Using this method of implant treatment, four implants are placed in the upper jaw and two or four implants placed in the lower jaw.   As in the previous case, temporary prostheses may be provided immediately after implant placement. Compared to removable dentures, this treatment option allows to avoid covering the palate and other anatomical areas, therefore is well tolerated by the patients (no gagging, etc.). Chewing efficiency is greatly improved even with the smallest number of implants, compared to complete dentures.

Removable dentures on mini implants

This is the most affordable treatment option, since the implants used do not need a separate abutment (they come complete with the abutment). This method is also useful if a significant amount of bone is lost and conventional implants would require bone augmentation (surgical restoration) procedures. Mini implant diameter is very small (approx. 2 mm), therefore augmentation procedures are unnecessary. 

Such prostheses are stable and aesthetically pleasing, and temporary prostheses may be provided immediately after implant placement. They are easy to look after. This solution is not only cost-efficient, but also quick – the patient can have stable removable prostheses in a short amount of time.

However, mini implants are less durable, compared to conventional implants. As such, over time they may fracture. Also, mini implants may need to be replaced and the denture adjusted due to wear of the prosthetic part.
